If you have recently given birth in Cebu, you will need to file a live birth certificate to officially register the child’s birth. The process is relatively simple and can be done at the Civil Registrar’s Office in Cebu City.

A Certificate of Live Birth, or COLB, is created when a child is born to document all important details surrounding the birth. The child’s parents review and sign the COLB before submitting it to the Local Civil Registry Office where the birth took place. This is based on our experience. When we received the live birth of our child and was already signed by our doctor. I went directly to the office of the lawyer near City Hall to have it notarized for Php 200, And after that, we went to the Office of the City Civil Registrar to have it certified.

Upon entering the building of the Office of Civil Registrar the staff gave me a priority number and told me to go to the 2nd floor, I saw a list of requirements for the certification of live birth.

Requirements

Below is the list of requirements for certifying the live birth. After gathering the requirements. I paid Php 500 for the fee for processing the document on the 2nd floor. It only took 2-3hours to get the certified live birth.

  • 1 Photocopy of the father’s Valid ID.
  • Copies of Livebirth (to be provided by the hospital).
  • Livebirth should be notarized by the lawyer

Registration Step by Step

1. Obtain the necessary documents. To file a live birth certificate, you will need to have the child’s birth certificate, the mother’s ID, and the father’s ID. Make sure u have a photocopy of the IDs and that the birth certificate has been notarized by the lawyer.

2. Go to the Civil Registrar’s Office. Once you have all of the required documents, you can go to the Civil Registrar’s Office in Cebu City to file the live birth certificate.

3. Fill out the application form. When you arrive at the office, you will need to fill out an application form for the live birth certificate.

4. Submit the required documents. Once you have filled out the application form, you will need to submit all of the required documents.

5. Pay the filing fee. There is a Php 500 filing fee that must be paid to file the live birth certificate.

6. Wait for the birth certificate. Once the live birth certificate has been filed, you will need to wait for it to be processed. This can take a couple of hours.

7. And after that, it is done. Double-check the signature of the medical records before leaving the office.

Reminders

  • Be advised that the office is only open during weekdays during government working hours Monday to Friday 8 am to 5 pm.
  • Bring money to pay for the fee.
  • Please practice social distancing.
  • Have all your requirements and payment ready to avoid taking time in the queue.
  • Don’t forget to have a copy of your receipt once you’ve settled your payment!
  • Review the data on the prepared certificate of live birth, focusing primarily on spelling mistakes for the child’s name, sex, and date of birth.

For Delayed Registration

An affidavit for delayed registration must be completed by one of the child’s parents or guardians to explain the reason/s for why registration has been delayed. The Certificate will only be issued 10 days after the affidavit has been posted.
