To apply for the Maternity Package, pregnant women should visit their local SWUMC facility and fill out an application form. The Maternity Package can provide expecting mothers with coverage for physician visits, lab tests, ultrasounds, and other related services. In addition to covering medical costs associated with childbirth.
Guidelines for Applying the Maternity Package
SWU Medical Center offers Maternity Package for 28 weeks (at most) pregnant women for both normal and CS delivery. When applying for the Maternity Package, expecting mothers should provide proof of pregnancy. This may include a signed form from their doctor or midwife, an ultrasound image, or a completed application form with all required information.
As soon as you avail of the package, you will be required to pay Php1,000 for Normal Spontaneous Delivery (NSD) and Php1,500 for CS as the initial payment. The remaining amount will be paid for every visit thereafter. A negative RT-PCR test result will be required for admission and delivery and they will also require you to donate one (1) bag of blood before giving birth. Please visit SWUMC Reproductive Health Unit (RHU) to avail yourself of the package. RHU is open Monday to Friday, 8 am-2 pm; Saturday, 8 am-12nn. For your first visit/consultation, you will be charged Php100.00.

A: You can deposit a minimum of P500.00 every visit and the remaining balance of the package will be settled upon discharge.
9. Can I choose the type of delivery?
A: No, the type of delivery will be determined by the attending physician depending on your condition.
10. What if I want to transfer to another hospital?
A: You may transfer to another hospital subject to the approval of your attending physician and upon payment of all fees due. However, if you have already availed of the Maternity Package, there will be no refund or transfer of credits. The package is non-transferable and non-refundable.
