Requirements to Avail the Maternity Package

There are certain requirements for those who would like to avail of the Maternity Package. These are the following requirements:

  1. 32 weeks of gestational age are accepted.
  2. Laboratory results and ultrasounds from previous prenatal. If no previous laboratories, they only accept up to 28 weeks of gestational age.
  3. One valid ID for the patient and one valid ID of the partner.
  4. 2000Php for the enrollment fee.
  5. A donation of 1 unit of blood is required before giving birth.
  6. A negative result of the RT-PCR test is required starting 37 weeks AOG and during admission (5 days result in validity).

Please take note that there will be no more orientation since the start of the pandemic.
