- Original Certificate of Registration (CR).
- If CR is not available, please submit an affidavit of loss.

Application Process: Step-by-Step
Here are the steps to follow for storing your motor vehicle at your preferred LTO Office:
Step 1: Visit your preferred LTO District/Extension Office to process the storage transaction.
Step 2: Proceed to the Customer Service Counter to have your requirements checked for completeness.
Step 3: Obtain an Application form for the storage of motor vehicles.
