- Letter Request
- Authorization letter and a photocopy of a company-issued ID with a photo and signature should be provided if the requesting party is unavailable.

- Letter of request
- Supporting legal documents (Police Report, Court Order, Subpoena, Filed Proceeding, or Motor Vehicle owner’s consent)
- Company Identification Card / Original and one (1) copy of any issued ID with photo and signature of the requesting party and the Motor Vehicle owner.
